In support of our vision and values, Island Health has developed a medication safety strategy that will ensure that safety is embedded in all aspects of medication management.

Safeguards in medication systems and processes reduce the risk of medication errors at the three main points of the medication process: ordering, dispensing and administration. 

Our vision is to make Island Health an example of best possible practice in ordering, dispensing and administering medication. By implementing a regional, closed-loop medication system, we will create an environment where clinicians have access to the necessary tools and systems for safer medication management, as well as the ability to initiate improvements based on comprehensive reporting and follow up. Access to both individual clinical data and system-wide reports will support these quality activities. Island Health's medication safety strategy will change the culture and practice across all programs and all locations to improve the safety of our patients and positiion Island Health as a leader in patient safety.

Current projects to improve medication safety include regional implementation of Pharmacy Order Imaging and Bar-code Unit Dose Packaging. These are foundational elements of the medication safety infrastructure which will support a new closed-loop medication administration system.

Recent improvements have been realized in removing high concentration electrolytes from care areas, significantly improved medication reconciliation in Residential Care, increased use of regional clinical order sets and building stronger relationships with the Professional Practice Office through Care Delivery Model Redesign work and overall support to the Medication Safety Strategy. Committee structures have been refreshed and an integrated focus across clinical practice, pharmacy and Information Management and Information Technology continues to be more fully established.
